In the three-dimensional planning of orthodontic treatment, the transverse problem should be prioritized. When a skeletal transverse maxillary deficiency is diagnosed, rapid maxillary expansion is the preferred treatment. A thorough understanding of its scope and limitations— considering the patient's age, the stage of facial suture closure, and appropriate anchorage —leads to proper execution and optimal results. Nowadays, thanks to two advanced tools, cone-beam computed tomography (CBCT) and microimplants, the age range for applying this technique has been broadened. CBCT allows for a three-dimensional diagnosis of the midpalatal suture, indicating its ossification degree and quantifying transverse discrepancy from stable bone points. The use of microimplants provides biomechanical advantages by minimizing unwanted tooth movement due to osseous anchorage, resulting in more effective and efficient treatment. This article reports the case of a 17-year-old male patient diagnosed with a collapsed maxilla and a stage C intermaxillary suture according to the Angelieri Method (AM), treated with a metallic device with an expansion screw anchored to two palatal microimplants, achieving a positive widening of the maxilla.References
